In preparing for your child's baptism, please complete the form below to tell us which form of baptism your child would like to receive.

For children who will go into the Baptismal Font, they should come to church in shorts and a t-shirt. We will have something that they put on over their shorts and t-shirt. After their baptism, they will have time to change into their Easter Clothes, while the rest of the parishioners are renewing their baptismal vows. NOTE: The Church provides a towel to use immediately after the child is baptized, however, you might want to bring an extra towel for your child to put in the changing room to use as they are putting on their Easter Clothes.
