Each page must be completed in its entirety in order to submit. Electronic upload of current physical required. These forms only need to be completed once each school year, even if your student plays multiple sports.Click HERE to complete the fillable CYO Consent Form
Please complete this form, print it out and return it to the school office if youdd not do so on Verification Day. This form only needs to be completed once each school year, even if your student plays multiple sports.
Documentation of a current physical must be on file prior to a student participating in any athletic tryouts, practices, or games. Click this link -PHYSICAL FORM- for the accepted IHSAA physical form. Pages 1 & 2 are filled out by the physician conducting the physical. Upload the signed physical examination form to the Athletic Forms (Step 3) before the first sport is played each year. Valid physicals must be dated after April 1 of the previous spring.
